Bis auf

http://bugs.otrs.org/show_bug.cgi?id=1189

ist jetzt alles gefixed bzw. hat es wie vorgesehen funktioniert und die 
Reports waren keine Fehler sondern haben irgendwelche anderen Ursachen.

Bug #1189 ist leider nicht so einfach zu fixen, dafür müsste IMHO das 
gesamte System umgebaut werden, das die Notifications zusammenbaut.
