[otrs] Attachments not uploading in the FAQ module (v1.0.6)

Hello all!
I have recently upgraded from v2.0.4 to v2.1.1.
I encountered an issue with attachments not uploading in the FAQ module (v1.0.6) which came out just a day to two ago).
The system log reports the following error:
 
Sun Oct 8 07:20:41 2006 error OTRS-CGI-01 Unknown column 'created' in 'field list', SQL: 'INSERT INTO faq_attachment (faq_id, filename, content_type, content_size, content, created, created_by, changed, changed_by) VALUES (39, 'OTRS TEST-UPLOAD-FILE.txt', 'plain/text', '3723', ?, current_timestamp, 1, current_timestamp, 1)'
 
The file did not attach or upload. - It looks like a bug . . if so how do I report it?
Any ideas?
