I guess you installed the rpm from Fedora Extras then. You can check
this by running 'rpm -qi otrs'.

Probably the default setup limits the access to localhost, which is
not a bad thing, because as long as the default admin account exists
with the default password anyone could take over your OTRS
installation. The idea the Fedora Extras packager had was probably
that you first change the admin account. I don't know where the
Fedora Extras rpm puts the apache config for OTRS though. Did it add
a conf file in /etc/httpd/conf.d/?
