I had some strange issues similiar to your's when upgrading ITSM packages. Now i am upgrading like that:

- set file permissions

- restart apache

- install one of itsm packages (GeneralCatalog, this one must be the first)

- again set file permissions

- again restart apache

- install another itsm package (ITSMCore, this one must be the second)

...

This looks strange, but it works.. By the way I use gentoo linux and source install of otrs. If you are using one of rpm distros and use rpm install, things are different.
