I've recently had to work through a very similar issue. I had everything configured correctly but I got the same error as you when trying to log in customers.
What I found was happening was that the ldap authentication suceeded but it wouldn't sync the data elements from the ldap tree to the database.
What I did to fix it, with the help from others here, was to use a very basic configuration for customer ldap authentication.
Here's a link to the conversation thread I had which eventually lead to the fix. The link is to the middle of the thread where Shawn gave me a basic config that worked for him and also worked for me when I used it. I then enabled each extra setting one by one after that and it all worked.

Yes, it means your user list doesn't match your authentication list. The Wiki and the documentation and the forums all have good information on this.
