"Message: MySQL server has gone away" You MySQL server isn't able to store the attachment, because it's to big for your MySQL server. Change your max_allowed_packet (default 2MB) in /etc/my.cnf or switch to the ticket-filesystem backend. See also: http://doc.otrs.org/1.1/html/performance-tuning.html#PERFORMANCE-TUNING-OTRS...
