Well, actually memcached DOES have perl support; an example is here on this
page: http://www.danga.com/memcached/
One of the biggest social network sites in The Netherlands makes heavy use
of memcached which made me also think about potential benefits for using
memcached in combination with OTRS. As Martin stated, it might be rather
trivial to implement OTRS support for memcached because it would be 'just
another' caching backend. The question would be if there is a benefit in
using memcached over the current caching implementation. So load testing and
performance measurements should are really important.
And also, it would be rather important to find out if in the current
implementation caching is implemented in the right places. If there would be
developers or OTRS admins currently working in heavy load environments (the
typical places where you'd benefit the most from caching I guess) that would
be able to do some measuring and testing and provide feedback on this, that
would be great!

maybe it's a better issue for the developer mailing list... next time. :)
There is a cache object in OTRS available with currently one backend (file system). It is totally easy to write a backend for memcached.
