What does the log file say? Which log file should I examine ?
Your logfile is defined in Kernel/Config.pm, or you might be using Syslog. I'm having a closer look at this issue and will later come back to it. In the meantime I would like you to file a bug report on http://bugs.otrs.org/ providign as much information as possible.
