It does find a match, and sets X-OTRS-Queue to spam. That's it..
Does the default OTRS check on X-OTRS-Queue?

You use OTRS 1.2? If yes, try to use "bin/PostMaster(POP3).pl -d2" and
you will find debug infos in your OTRS log.
E. g.:
[...]
Jan 30 15:25:40 cuba OTRS-PM-10[5018]: [Debug][Kernel::System::PostMaster::Filter::Match::Run][70] 'System Tester I ' =~ /noreply@/i matched NOT! 
[...]
