I know this problem... I suffered it while setting up mod_perl2 on Debian. Some people (myself included) have posted here some replies to you regarding how to setup Debian with mod_perl2. Have you followed those instructions?
I'm guessing that this is a problem in apache, but I'm not positive. I get it when I try to log into the system, otherwise it seems like I can get to index.pl and installer.pl fine. Although I don't need to get to installer.pl. I just tried playing with installer.pl, and I can't even delete/create the database. It seems like there might be more going on than I think. Oh, this is for the otrs 2 beta 4. Does anyone have ideas about this?
Please, check this thread (specially a reply that Christian Schoepplein did to me, clarifiying some important things) and try again: http://lists.otrs.org/pipermail/otrs/2005-July/008313.html I'm sure we'll get it to work, sooner or later! ;)
