Changing "otrs@mydomain.com" to simply "otrs" did the trick. My qmail is adding the "@mydomain.com" . I've added a .qmail-support entry in otrs@mydomain.com with "| /var/qmail/bin/preline procmail" in it and I'm now getting my mail processed into the otrs system.
