That should be fine. Just edit the file /opt/otrs/var/cron/fetchmail and remove the comment from line 13. Save the file and restart OTRS with the command rcotrs restart Now a new crontab with the activated fetchmail job will be installed and your mails should be fetched. Because the cronjob runs for the otrs user your settings in ~otrs/.fetchmailrc file will be used.
