Can it be you have (had) installed mod_perl1 for Apache1 some time ago? If so, make sure it's completely gone.
Was almost sure... So I did a clean install of Mandrake 10.0 with minimal package selection in vmware, and no system updates!!! After a quick install of OTRS all seam fine with both cgi-perl and mod_perl. I guess I will have to clean up my web server.
