you can try this (easy way to install)
disable your apache en mysql, backup your apache en mysql folder
remove the original folder
install otrs win32 rpm in the in te same location as apache and mysql was
installed (example : c:/webserver/)
run complete install untill otrs is working
backup this folder
remove the original folder and replace with backup of your apache and mysql
copy the subfolder OTRS from the OTRS backupped folder
install strawberryperl with needed modules, or just copy it from the backup
otrs install
edit the config files in apache (the httpd.conf) with the info you find in
backuped otrs install - otrs - scripts
apache 2 and perl . pm
run apache and mysql, go to localhost/otrs/index.pl or so and watch ;)

I only find an old article for ver 1.3 (http://doc.otrs.org/1.3/en/html/install-win32.html), discussing the Windows installation. FAQ shows nothing. (sigh)
Yes you can but Apache will not start up, you will need to modify the Apache configuration (not sure which directory it is installed to on Windows) and change the port it is running on (as port 80 will be in use by IIS) so you could probably change it to port 81 and then goto http://machinename:81/otrs to access OTRS.
