See chapter 8.4. "Customize the customer self registration". This part of the manual explaines how to add a additional phone field to the customer frond-end and the database. You can add other fields the same way.
Optional i want to disable customer login
Just set the customer you don't want any longer to login to "invalid" via the agent interface.
