I've seen this might be related to some default templates partially installed or usage of mod_perl vs mod_perl2. Thanks for any guidance on this.

How have you installed OTRS? Normaly you should have some templates for responses that are already stored in the database. If you have created the database manualy, the initial data are taken from the initialinsert.* scripts.
Otrs is currently running on RedHat AS 3, with perl and apache + mysql.
Have you taken the webinstaller to setup your database? Somehow it looks like your system is not installed completely.
Can you tell me more about your system? What version of mysql, apache and perl are you using? Are you using mod_perl2?
