Robin, The best way to do this is to copy the /Kernel/Output/HTML/Standard folder naming it what you'd like it to be then modifying it to suit your needs. You should be able to add a theme by running an insert statement into the themes table. Is there a way that I can easily change the icons that appear at the top header to appear in a vertical arrangement on the left hand side ?

Assuming your running OTRS 2.0.x you can simply copy modified files in your new theme folder. The System will check wether a customized file excists in that folder. Otherwise it will use the equivalent file from the Standard-Theme. :-)
Is there a way that I can easily change the icons that appear at the top header to appear in a vertical arrangement on the left hand side ? I don't think it's that easy but I would love to get notified if you succeeded. :-)
