A form per queue. Is it possible?

I would like to provide my people with a group of few (4 or 5, no more) different web form templates designed to fit needs of each particular queue. In particular I would like to create specific forms adding to them different custom fields for different queue. For instance, if my people needs to fill a ticket to request to move a PC from the soket1 to the soket2, I want provide them a form (template) equipped with a field to enter the ID of the soket1 and the ID of the soket2 (plus name, building and so forth). If they ask to change the PC I want to provide them a form (template) equipped with only the field to enter the ID of the soket1 (soket2 is useless in this case) (plus name, building and so forth). What I am looking for is a way to add custom field cf01 and cf02 to the form for queue q01 and cf03 and cf04 and cf05 to the form for queue q02 and so forth. Up to date I was able to add custom field to the UNIQUE form I can provide for my customers. What I am looking for is a way to create different forms for customers and distinguishing them by queue. How can I do this?
I don't think it is easy to extend OTRS to support this. The easiest solution I see is to set up a separate webpage that has links to the different forms for the different buildings and let those forms mail the output to the address of the associated queue. Nils Breunese.
