What I am looking for is a way to create different forms for customers and distinguishing them by queue.

The easiest would be to create forms in whatever web programming language you like and have them submit mails to OTRS. You can set a slew of headers in these mails which will guide them to the right queue, set priorities and so on. -- Lars
