I just use the "Standard" theme and mine looks just like the doc's too. Click the "Preferences" link and make sure your "Theme" is set to "Standard". If it is set to Standard yet your screens still don't look like the demo then I'd look into the possibility that someone edited some or all of the .dtl files in "otrs\Kernel\Output\HTML\Standard\". If that has happened, the original files should be easy enough to retrieve. Worst case, install otrs on another system then copy the files over to your existing box.
