But if the database is already in place, why would I need to re-run the

Yes, you need to run all applicable database scripts (in order). See the UPGRADING file. We don't run Debian, but if it doesn't work then you might want to file a bug against that package.

The database schema has changed from 2.0.4 to 2.2.4. The scripts modify the database so it works properly on the new version of OTRS. You cannot use a OTRS 2.0.4 database with the OTRS 2.2.4 code. Nils Breunese.
