Your solution may, or may not, be simple. It could be configuration error in Apache (or simple file permissions) or if SELINUX is enabled on the system it could be blocking the access. (If you didn't disable it or set in permissive mode mode OS install with it enabled. OTRS is incompatible with SELINUX default setup.)
No specific order...1. Should confirm that apache is functional. apache logs2. Check SELINUX status and disable (SEstatus may help in this - Suggest researching exact command for SuSe)3. File permissions allow access (ls -l). OTRS has a file permissions script I. the script folder... I think it's otrs.spermissions but you will need to know the users and groups which run on your system.4. Check error logs (most are in /var/logs/)
The specifics are outside the scope of list as they pertain to the specific configuration of your system and the O/S running on it. I think the install document covers some of these items, in part. There are apache and Linux os lists which can help with the details.
Look into your system logs and find error notices. They should direct your steps.
