If your Application and MySQL servers are on different boxes check to ensure the MySQL server is listening on the correct IP address AND that you have the firewall open for the MYSQL port (TCP 3306) at least allowed for the application server. I think what it boils down to is the windows bundle installer wasn't written to use an external database... I would happily use the internal db server if there was a post installation migration path to the desired database server. The file is in the installation folder of otrs under Kernel\Config.pm

Hi Ryan,
There is a problem in the 'old' web installer where it creates an
incorrect user account on the database when using a remote database. I
corrected this in the web installer that is in the Windows installer and in
the upcoming OTRS 3.3. Best you can do is to manually create a correct
account in the MySQL database by hand, and put its credentials in OTRS's
Kernel/Config.pm.
