Could it be related to a ("high") number of item in configuration ? I mean:
users: 83
queue: 256
groups: 70
customer_user:107
(valid and invalid)
If the tool try to compose an update with so many item could pass a kinda
buffer so it does nothing (just a fantasy guess)?

I couldn't reproduce this with my OTRS 2.4.4 on OpenSuse 11.1 :(
I add an agent and then I can select groups. When I submit and check my work the agent exists and is in the selected groups.
