
Hi Hugh,
The problem is that you have a Content-ID in your email for this attachment
although it's not in-line. This causes some MUA's (Outlook) to 'hide' it.
This only occurs if you used WebUploadCacheModule::FS and has been fixed
already, see http://bugs.otrs.org/show_bug.cgi?id=6628

Interestingly, this works (application/octet-stream instead of application/pdf):
