I figured it out myself. For some reason, otrs didn't like the FQDN of my mailserver for the smtp settings, but rather wanted just the server name. I initially had mail.xxxxx.org as the host, but changed it to serverxxxxx, and now it sends just fine. You firewall probably doesn't work well with a loopback connection. well done...
